Household delivery consent form download

FREE/Non-Congregate/Home Delivered/Grab-n-Go Style Meals

·      Open to all children and teens up to 18 years of age and young people 19 to 26 years old enrolled in continuing and/or special education services

·      Breakfasts and Lunches for 7 days provided for each child/teen/youth every week

·      These meals are packed at the Schoolcraft County Fair Building and delivered to your home

·      Deliveries will be starting Tuesday, June 9^th.  Meals will be delivered on Tuesdays (3 Breakfasts and 3 Lunches for W, Th, F) and on Fridays (4 Breakfast and 4 Lunches for Sa, Su, M, Tu) throughout the entire summer until August 21^st

·      Deliveries will be made between 1:30 and 5:30PM every Tuesday and Friday. Once everyone has signed up, our delivery routes will be established and a more accurate delivery time can be determined

·      We prefer that someone be available to accept your food delivery but if you are unable to do so, we ask you to leave a cooler out for us, preferably with ice packs in it as some of the foods delivered are frozen or need refrigeration. Our delivery people have a have a limited supply of insulated bags and freezer packs

·      If you are unable to accept your food delivery you do have other options 1. Call or text (906)286-9185 and notify us you will pick up your food that day or 2. You can provide an alternate drop of location or 3. You can call or text (906)286-9185 to cancel your delivery for that day. Please be sure to notify us before Noon.

·      All household with children/teens/youth qualify. Just complete a Household Contact Information and Consent to Deliver Meals form that gives us permission and the information we need to make the food deliveries

·      Household Contact and Consent forms can be found on our website, www.schoolcraftfairgrounds.com or can be requested via email at [email protected]. After June 8^th you may pick up a form at the Schoolcraft County Fair Building, Monday-Friday 7AM-5PM!   

 

Congregate/Home Style Meals

·      Free meals served at the Schoolcraft County Fairgrounds

·      Starting June 8^th and open through August 21^st

·      Drop-in, congregate meal service. No enrollment necessary.

·      Open Monday through Friday to all children up to 18 years of age and youth 19 to 26 enrolled in continuing or special education services

·      If participate in weekday congregate meals, are also eligible for grab and go style meals for Saturdays and Sundays. These meals will be available on Fridays after 3PM.

·      Breakfast is served from 7:30-9:30 AM. Lunch is served from 11:30-1:30PM.

·      Join us for one meal or both breakfast and lunch
